    Cardiac development, by Lisa McCabe.

  • 00:27

Hello, my name is Lisa McCabe.

  • 00:29

I’m a clinical nurse specialist at Children’s Hospital Boston in the cardiovascular program.

  • 00:35

I will be discussing with you today Cardiac development.

  • 00:36

The information I swill share is consistent with our practice here at Children’s Hospital

  • 00:37

    Boston.

  • 00:38

You may want to adapt this information to your own institutional practice.

  • 00:39

    Fetal Development.

  • 00:40

During the first week of fetal life, the fertilized egg develops into a blastocyte and implants

  • 00:45

    in the mother’s uterus.

  • 00:48

During the second week of fetal life, the blastocyte implants deeper into the uterine

  • 00:53

wall, and a primitive placenta begins to form.

  • 00:58

During the third week of fetal life, the primitive umbilical cord develops.

  • 01:03

Also at this time, the blastocyte develops into a three-layered disk.

  • 01:08

The three layers are: the endoderm, mesoderm and ectoderm.

  • 01:15

Specific body systems will develop from each layer.

  • 01:20

The endoderm, or inner layer, gives rise to the primitive intestinal tube, mucous membranes,

  • 01:28

glands, lung buds, urinary tract, and yolk sac.

  • 01:35

The mesoderm, of middle layer, gives rise to the heart and vascular system, the dermis,

  • 01:42

subcutaneous tissue, muscles, skeleton, sex glands, lymph glands, kidneys, connective

  • 01:50

    tissue, and blood cells.

  • 01:53

And finally the ectoderm, or outer layer, gives rises to the epidermis, hair, sebaceous

  • 02:00

glands, sweat glands, and nervous system.

  • 02:05

    Cardiogenesis.

  • 02:07

Early in the development, the primitive heart develops two tubes that merge into one tube.

  • 02:14

The single tube begins to swell, and develops into various anatomic features of the heart.

  • 02:21

The heart begins o beat by week three.

  • 02:25

In normal cardiac development, the cardiac tube will twist and turns on itself in a rightward

  • 02:31

    direction.

  • 02:32

    This is called dextral-looping.

  • 02:36

This results in the right ventricle developing on the right side of the heart and the left

  • 02:41

ventricle developing on the left side of the heart.

  • 02:45

Abnormal looping in a leftward direction is called leval-looping.

  • 02:51

This results in the right ventricle developing on the left side of the heart and the left

  • 02:57

ventricle developing on the right side of the heart.

  • 02:59

    Atrial Septation.

  • 03:00

Entering into the fifth week of fetal life, the atrial septum and ventricular septum begins

  • 03:06

    to form.

  • 03:08

The atrial septum grows in layers, and includes the tissues of septum premium, septum secondo,

  • 03:15

    and endocardial cushion tissue.

  • 03:18

The endocardial cushion tissue is located in the middle of the heart.

  • 03:24

From this tissue arises the tricuspid valve, mitral valve, part of the atrial septum, and

  • 03:32

    part of the ventricular septum.

  • 03:35

Septum prium grows downward between the right and left atrium, and eventually fuses with

  • 03:41

    the endocardial cushion tissues.

  • 03:45

The septum secudnum grows parallel to septum primum.

  • 03:51

Both septum primum and septum secundum develop with holes in them to create a passageway

  • 03:58

for blood to flow from right atrium to left atrium through the foramen ovale.

  • 04:04

As long as the pressures on the right atrium are higher than pressures in the left atrium,

  • 04:11

the passage will stay open to allow the blood to flow from right to left.

  • 04:17

Errors may occur during atrial septation.

  • 04:21

A secundum atrial septal defect is an opening in the middle of the atrial septum, produce

  • 04:27

when the tissue of septum primum does not reach septum secundum adequately to completely

  • 04:34

close the wall or when a tiny hole is left in the septum primum as the tissues grow.

  • 04:42

A primum atrial septal defect is an opening in the lower part of the atrial septum, near

  • 04:50

    the tricuspid and mitral valves.

  • 04:53

It results from problems in the growth of the endocardial cushion tissues.

  • 04:58

A defect here is often associated with a defect in the mitral valve, as well as tricuspid

  • 05:06

    valve.

  • 05:07

The most severe form of this is an atrial ventricular canal defect.

  • 05:12

    Truncal Septation.

  • 05:13

The aorta and pulmonary artery develop from a single tubular structure: the truncus arteriosis.

  • 05:21

Two areas of thickened tissue project into the lumen of this tube on the right and left

  • 05:27

    side.

  • 05:29

As they continue to grow in size, these ridges meet and fuse to form the septum, which takes

  • 05:36

a spiral course toward the end of this tube.

  • 05:40

This septum divides the truncus into two vessels: the aorta and the pulmonary artery.

  • 05:48

The conal tissue involved in the septation of the truncus arteriosis also directs placement

  • 05:54

of the aorta and pulmonary artery over their related ventricles.

  • 05:59

    Ventricular Septation.

  • 06:00

By the fifth week of fetal life, the atria completely separate into the right atrium

  • 06:06

    and left atrium.

  • 06:07

Also at this time, the ventricular septum continues to develop.

  • 06:13

The muscular portion of the ventricular septum develops from the apex of the common ventricle.

  • 06:20

The membranous septum develops from the endocardial cushion tissues.

  • 06:26

Conal tissue contributes to the complete closure of the septum between the aortic and pulmonary

  • 06:32

    valves.

  • 06:44

Errors may occur during ventricular septation.

  • 06:48

Muscular defects can occur in any portion of the muscular septum.

  • 06:53

Small defects often close on their own as the tissues develop.

  • 07:01

Membranous defects are located behind the septal leaflet of the tricuspid valve.

  • 07:10

Sub-pulmonary defects are caused by deficiency of the conal septum.

  • 07:16

Atrial ventricular canal type defects are caused by defects in the endocardial cushion

  • 07:23

    tissues of the intraventricular septum.

  • 07:29

    Arch Formation.

  • 07:32

The aorta and pulmonary artery and their branching vessels are formed from the brachial arch

  • 07:38

    arteries.

  • 07:40

Initially there are six pairs of brachial arch arteries.

  • 07:45

The first, second, and fifth pairs disappear, forming ligaments that hold the heart in place.

  • 07:51

The third aortic arch forms the common carotid artery, external carotid artery, and internal

  • 07:57

    carotid artery.

  • 07:59

The fourth aortic arch forms part of the final aortic arch, as well as the proximal portion

  • 08:06

    of the right subclavian artery.

  • 08:09

The sixth aortic arch produces the proximal segment of the branch pulmonary arteries,

  • 08:17

the ductus arteriosis, and provides pulmonary blood flow via the branch that develops lung

  • 08:25

    buds.

  • 08:26

Errors may occur in arch formation that result in the following defects: coarctation of the

  • 08:34

aorta, interrupted aortic arch, aortic atresia, patent ductus arteriosis, and vascular ring.

  • 08:44

In just a few short weeks, the human heart has undergone major development.

  • 08:49

By the third week of fetal life, the heart is beating.

  • 08:52

BY day 44, the fetal heart resembles the postnatal heart in structure and function.

  • 08:59

It will continue to grow and develop over the next 30 to 32 weeks.

  • 09:06

Many of the features that may results in a heart defect, however, have also begun to

  • 09:13

    develop.
